Trinity Health Of New England is looking for a Director of Case Management (RN) to join our team of mission-driven healthcare innovators at Saint Francis Hospital. This person will direct, plan and lead all activities in our Case Management Department.

You Will Be Responsible For:
  • Exceptional Leadership: Plan and lead the all activities of the department from ensuring use of the Healthcare Value and Quality Platform to improve operational efficiency as well as supporting team and individual colleague growth.
  • Creative Problem Solving: Brainstorm with colleagues to design and implement programs that monitor the delivery of patient care to ensure optimal quality, cost-effective medical and specialty services; appropriate discharge disposition; and prioritization of the allocation of hospital resources.
  • Optimization of Resources: Maintain and monitor operating statistics for the Department adjusting staffing, supplies or capital expenditures. Recommend the annual capital, salary and non-salary budget for the Department of Care Management.
  • Technical Familiarity: Understanding in computer-based applications (E-MAR, clinical documentation), email, Proficient in MS Word, Excel, and PowerPoint; Trained in MS-Project or use of Excel for project management.

Requirements:
  • Education: Bachelors in Nursing - Master's Degree Preferred
  • Licensure: Current Licensure as a registered nurse (RN) in Connecticut
  • Experience: Five or more years of progressive experience in a hospital / health system setting in a leadership role in case management or a related role
  • Must be knowledgeable regarding state and federal health regulations, reporting requirements and health care reimbursement systems.
  • Basic knowledge and application of statistical analysis, DRG's/ICD9/10, Medical Necessity criteria, JCAHO and CMS requirements for discharge planning and utilization management, levels of care in continuum and dynamics occurring in health care delivery.
  • Ability to pass drug screenings and background checks

Preferred Skills:
  • Certification: Certification in Case Management (ACM or CCM)
