POSITION SUMMARY:

This position conducts the administrative segment of the admissions intake process.

POSITION SPECIFIC RESPONSIBILITIES:

• Facilitates the overall intake process, coordination, and census management.

• Creates, edits and keeps documentation at all stages of the intake process, including but not limited to administering and filing intake forms and recording the individual's information.

• Verifies eligibility and updates eligibility status accordingly.

• Makes appropriate connections/linkages to internal and external resources

• Conducts interviews as a part of gather information needed

• Provides information and referral services and assist individuals/families/parents in making necessary informed choices with available options

• Arranges assessments and appointments.

• Ensures information into systems, files and/or databases is accurate and timely

• Collaborates inter/intra agency team regarding individual requests and referral agency

• May compile information from a variety of industry specific software programs and resources to create, compile, maintain and distribute reports/data and documents using Microsoft Office programs at an intermediate level, or proprietary software, in a timely manner.

• Coordinates and follows up with team in order to meet established deadlines.

• Other duties as assigned as it pertains to the position

POSITION REQUIREMENTS

Bachelor's Degree required in Human Services related field.

2+ years of experience with social services/healthcare admissions.
