The position is grant funded for a minimum of (four) years.

Position Details

The Certified Community Behavioral Health Clinic (CCBHC) Nurse Navigator is a Licensed Practical Nurse or Registered Nurse. As such, this staff member is uniquely trained to consider the interplay of the person, environment, and health. The primary role of the Nurse Navigator is to facilitate screenings, assessments, and vitals collection. The Nurse Navigator also functions as a health advocate to aid individuals in receiving the physical health and behavioral health care they need, in addition to assisting in coordinating care and the development of personal plans for consumer health collaboration and wellness.

Responsibilities include but are not limited to the following.

• The Nurse Navigator will provide community outreach, education, and other support services to the targeted populations,

• The Nurse Navigator will support Certified Community Behavioral Health Clinic (CCBHC) grant requirements through completion of National Outcome Measures surveys and vitals data collection.

• Will be responsible for communicating CCBHC grant project participation expectations and benefits to consumers, including completion of consent to participate and other forms as needed,

• Will provide coaching and mentoring to identified consumers,

• Conduct routine assessments/screenings and reassessments as assigned,

• Complete service notes and other documentation of services provided,

• Provide integrated service coordination and communication for individuals served, including coordinating and monitoring behavioral and physical health care,

• Provide Physical and Behavioral Health support and education to consumer about his/her health conditions,

• Coordinate services with other members of the Merakey services team as needed,

• Develop a Wellness Plan with the individual as necessary,

• Assist consumer in identifying wellness goals and action steps for how to achieve those goals,

• Collaborate and communicate with other healthcare staff involved in consumer's care. Complete and coordinate necessary referrals. Provide follow up to assure that recommended services are provided,

• Coordinate care with other Merakey staff to ensure consumer has access to Mental Health, Substance Abuse, Primary Care, and specialty services,

• Review member profile, psychosocial and medical history,

• Be the liaison between family and other natural supports and provide information and education as permitted by consumer and as needed/applicable,

• Coordinate appointments with Merakey staff within 7 days of an individual's discharge from an inpatient setting,

• Complete all needed documentation in a timely manner as defined by regulations, Merakey policy and program guidelines. This includes all assessments, releases of information, consents, and progress notes for all completed and attempted interactions with the consumer, on behalf of the consumer, or for coordination of the consumers care,

• Attend supervision, team meetings, and trainings as assigned.

• Follow all Merakey's policies and state regulations.

The ideal candidate will possess one or more of the following qualifications:

• Licensed Practical Nurse (LPN) or Registered Nurse. High School Diploma. Bachelor's degree preferred.

• At least 1 year experience monitoring patient vital signs and overall health condition.

• At least 1 year experience in mental health setting preferred.
