Judge Healthcare is actively recruiting for Nurse Practitioners to work directly for one of our clients in Lititz, PA on a permanent PRN basis doing home health assessments!
This job will have the following responsibilities:
- Responsible for conducting home health visits on assigned members
- Reviewing patient's health status and making clinical recommendations as necessary
- Work collaboratively with family members and other healthcare professionals including PCP, case managers, social workers
- Responsible for patient education
- Additional duties as needed.
Qualifications & Requirements:
- Must have an active PA NP licensure in good standing
- Must be Board Certified
