Job Summary:

Bay Cove Human Services/CASPAR Inc is a leading provider of housing services for people who are experiencing homelessness, who are at risk of experiencing homelessness, or who have recently moved from homelessness into a stable housing situation. The role of the permanent supportive housing case manager is to support individuals in making the transition to housing and to maintain their housing using a person centered, collaborative, case management approach.

Reports To: Director, Homeless Services, A&HS

Essential Functions of Position:

•Engage program participants in case management services focused on retention of permanent stable housing.

•Complete intake and assessment interviews to determine participant needs upon referral to the program.

• Work with tenants to create client-centered, strengths-based, individual service plans (ISP), to be periodically evaluated and edited as necessary/appropriate

• Actively manage a caseload of formerly chronically homeless tenants in scattered site housing promoting housing retention, income maximization, and greater self-determination.

•Provide high quality, culturally sensitive, client-centered intensive housing based case management utilizing evidence-based practices •

•Educate, coach, and reinforce independent living skills including, but not limited to personal hygiene, budgeting, cooking and leisure time activities. Assist tenants in shopping, laundry, and other ILSs as needed or requested.

• Provide strengths-based interventions with tenants experiencing psychiatric distress or other acute crisis. Develop individually based crisis intervention plans with each tenant in order to reduce the number of hospitalizations and/or police response. Intervene directly and/or cooperate with team efforts around crisis stabilization for tenants.

•Provide assistance and advocacy for participants working to secure benefits such as SSI/SSDI, Veteran Disability, SNAP/EAEDC, unemployment, health insurance, and other related services, as well as secure employment

•This position will be community based, with the majority of time spent working with housed individuals in their unit or assisting them in their neighborhoods.

•Carry out housing stabilization related activities including but not limited to helping with accessing furniture resources, moving services, and back utility bills.

•Assisting individuals in maintaining their unit.

•Providing the necessary support for housed individuals to adjust from houseless to housing

•Maintaining regular in person meetings with housed individuals in their units to observe how their living status is progressing and to report successes and obstacles that housed individuals have.

•Assist clients with obtaining documentation necessary for accessing housing opportunities (IE. birth certificate, photo ID, income verification, etc.)

•Provide financial assistance from various financial resource programs (both internal and external) to facilitate housing placements.

•Complete all paperwork, statistics, and data entry in a timely and thorough manner.

•Coordinate housing search work through regular meetings with members of CASPAR Housing Search Team.

•Participate in the agency's Walk-In Program, providing brief consultations to those seeking housing search assistance

•Complete timely assessments and service plans in collaboration with the person served

•Document all interactions in HMIS/Clarity

•Must have a valid Massachusetts Driver License

•Carry out other related duties as assigned.

Requirements for the position:

  • One-to-three years of housing search experience or related social service experience, preferably with single adults experiencing homelessness.
  • Knowledge of public, subsidized, and private housing resources preferred.
  • Demonstrated ability to learn technical information (policies and regulations) necessary to conduct effective housing search.
  • Knowledge of various mental health conditions.
  • Experience working with adults who have a substance use disorder or mental health conditions preferred.
  • Good problem-solving skills.
  • Patience and persistence in assisting people with multiple housing search barriers.
  • Willingness to be flexible according to client needs.
  • Strong interpersonal skills and ability to work well with diverse groups of people.
  • Strong advocacy and outreach skills.
  • Dedication to helping our clients gain self-confidence and setting goals that ultimately move toward exiting shelter and becoming successfully integrated into their community.
  • Willingness to travel to shelter sites and accompany clients to housing appointments, including viewing apartments.
  • Ability to work independently as well as part of a team.
  • Strong written, verbal, and math skills necessary.
  • Basic computer skills required.
  • Bachelor's Degree required
  • Retains full vaccination status for COVID 19 and emerging variants.
  • Work hours outside of 9am - 5pm may be required.

  • A COVID-19 vaccination is a requirement of the position. One COVID-19 shot is acceptable, contingent on the individual receiving the second shot within the allotted time frame.
