This highly specialized position works as part of a multi-disciplinary transplant team, to provide individual counseling or referral for individual counseling as indicated by patient's assessment. Works collaboratively with the behavioral health providers to include psychiatrists, psychologists and neuro psychologists for coordinated psychiatric patient care. Provides biopsychosocial assessments, individual/family/group psychotherapy as needed as well as case management services as required. This position is to support the referrals from providers in the ambulatory setting seeking mental health, social and financial services for their patients.

Perform Comprehensive initial psychosocial evaluation and documentation for each patient's candidacy for transplantation or living donation. Perform updates to existing psychosocial evaluations and reports as needed. Develop patient's plan of care based on team's recommendations. Develop and implement appropriate hospital discharge plans. Obtain prior authorizations as it pertains to post-operative transplant care. Participate in case management and discharge process of all transplant inpatients. Provide individual counseling or referral for

individual counseling as indicated by patient's assessment, Other counseling includes: Crisis intervention, death, dying and bereavement for patient and family, patient and family advocacy, patient and family education for post transplant coping, Substance abuse and smoking

evaluation and referral for appropriate treatment and monitoring. May work in concert with the ambulatory behavioral health team and other ambulatory providers. Organize and run

monthly support groups. Is responsible for the development and coordination of said groups. Identify opportunities for outreach. Understanding and communication with Community Services, Knowledge of local Social Service resources, Participate in community organ donor

awareness programs and groups. Participates in Transplant and system wide performance

improvement and quality initiatives. Maintain appropriate charting documentation per UNOS rules, Loyola and Department Policy. Documents in EPIC all pertinent patient information by the close of each business day. Properly codes and submits bill for relevant and approved services. Reports potential risk or quality management issues to department

manager of director. Maintain current knowledge base of clinical social work and utilization standards. Maintain current knowledge of UNOS rules and CMS conditions of participation as they pertain to transplant, knowledge of all post-acute care options including: palliative care, rehabilitation, skilled nursing homes, home health and home infusion. Knowledge of current Medicare and Medicaid regulations as they pertain to transplant, Knowledge of prescription drug benefits for commercial and governmental payors including Medicare part D, Knowledge of changing health insurance programs, Knowledge of local and national financial resources and assistance (including Prescription drug assistance), Adheres to Patient's Bill of Rights, Social Worker's Code of Ethics and professional responsibilities outlined by NASW. Seeks opportunities to meet learning needs through seminars and literature, including mandatory education requirements. Demonstrates capability to adapt and change work processes and role responsibilities or activities to improve service delivery. Establish effective networks with colleagues inside and outside LUHS. May mentor Social Work students as a field instructor in conjunction with the Department Clinical Coordinator and the specific University liaison. Participates in pertinent professional education to maintain certification, Participate in community transplant advocacy groups. Participate in multidisciplinary inpatient rounds and weekly medical review board meetings, Knowledge of UNOS rules and regulations and CMS Conditions of Participation pertaining to transplant.
